APPLICATION for those currently incarcerated
	Thank you for your interest in Twelve Step Education Program. We have sober homes in Billerica, Leominster, Saugus, Quincy, and Woburn.
	The application should be filled out and returned with a $130 non-refundable application fee (which will be applied to your fee upon admission) to our corporate office at: 55 Cambridge Street Suite 101 Burlington, MA 01803.
	The total amount due upon your arrival is $390.00 ($260 advance fee plus one week program fee of $130) The $130 application fee will be applied toward your $260 advance fee upon arrival. We cannot put you on our housing list until we have received your application and application fee.
	ABSOLUTELY NO CASH WILL BE ACCEPTED FOR ANY FEES AT ANY TIME
	All payments must be paid by money order or check to Twelve Step Education Program
	Parolees please note: Final approval is contingent on the field parole officer/parole officer’s review of your record along with our sober home policies. Your acceptance into the Twelve Step Education Program requires that you make a six month committment to our program during which you will abide by all house rules.
	Please note: we cannot accept anyone who is taking any mind altering substances such as (but not limited to) opiates, benzodiazapines such as Zanax, Valium, Librium, Ativan, Klonipan or any other medications that are mind altering.
